Tom tragically died aged 26, on Sunday, March 27, just 24 hours after he played in Reds match against AFC Rushden and Diamonds.
He had been with the club for three seasons including a spell as captain last year.
Tom had also played for near neighbours Bromsgrove Sporting and devastated fans held a minutes applause for the popular figure when the two clubs clashed a week after his tragic death.
His funeral service is this Monday, April 25 at 2pm.
The club said space is limited inside the chapel however additional screens and speakers will be used for those outside.
It added that flowers are welcome however donations at both the crematorium and Redditch United would be preferred for the Kaleidoscope Group supporting mental health and well-being.
Dress code for the service will be traditional black.
Following the service a wake will be held for Tom at the home of Redditch United, The Trico Stadium, Sallie Swan Street, B97 4AU.
